Canada Labour Code Part II, 127.1

HERE ARE TWO WAYS HOW:
Number 1:
* Obtain a H & S concern form- ACaeronet/my work/all forms/ACF32 or You may pick one up PDC

* Fill in the form and discuss it with a MANAGER. Have the Manager sign the form. The concern or issue will be investigated and the employee and the employer will be informed of the results.

*Make 2 Copies. The CSM who signed it keeps original, you keep a copy of the initialed H & S concern form for yourself, have a copy for the H & S office (drop it off to our office) and call 604-270-5234 to leave a message that you did so – so we can help track it.

Number 2:
Sign into the Portal. Once signed in at the Top of the screen, look for the tab SAFETY
from the drop down you will then see the link to
Submit a Safety Report/SIMS Depending if you are or not using the Air Canada network, you might have to sign in again. Once signed in you will then see
SIMS-Safety Information Management System
Look on the right for  New eReport and finally
Hazard Report

*The company must respond to your concern in writing within 30 days.

*If you do not accept the solution offered, the concern is then taken to a Health & Safety committee meeting for discussion and resolution.

* Still not satisfied with the response? The matter may be escalated to the Policy Committee or the Federal Labour Canada (ESDC)

Once you have finished your 15 week entitlement of  GIDIP and applying for EI Benefits ( Entitlement 15 weeks ) If you are approved to return to work prior to the expiration of your 15 week EI Entitlement, you must contact your GIDIP Case Manager in order to return to work. The reason you must contact your case manager is that they will forward any information regarding any modification/ restrictions to the  Air Canada on your behalf to ensure that Air Canada will accommodate your return to work.

WSIB Early Safe Return to Work Process Employee Information 

Injured at work? Air Canada is legislated to comply with the WSIB Early and Safe Return to Work Program.

In the event of an injury, the enclosed process must be adhered to:

1. Immediately report all work related injuries or illness to your immediate manager.

2. Seek medical attention and complete the WSIB Functional Abilities Form (FAF).

3. Report to the Resource Office (PDC) within 24 hours (or your next scheduled shift) of the injury to discuss return to work options.

4. Your completed FAF must be given to a Resource office manager. Failure to do so may result in your pay cycle being disrupted. In all cases, your claim will be adjudicated by WSIB.

5. All shift changes, overtime, undertime and vacation will be restricted until you have medical clearance to return to work.

6. Travel is restricted until you have been given clearance to return to work.

If you have any questions please contact your Resource office manager.

From the first fourteen (14) calendar days of your absence you will be retained on the Company payroll at full salary. From thereafter until you return to duty, you may be eligible for income protection under the (C.S.S.T/W.S.I.B/W.C.B.) provided you have obtained and completed the necessary form(s) from them.

During your absence, all approved shift trades are cancelled until your return to duty. Therefore it is your responsibility to make alternate arrangements for all shift trades.

Please note that retention on the payroll for the first fourteen (14) calendar days is dependant upon acceptance of your claim by the (C.S.S.T/W.S.I.B./W.C.B.). It is therefore necessary that your complete the form(s) regardless of the length of time you are absent.

It is also important that you seek medical treatment since retention on the payroll and Worker's Compensation benefits are normally conditional upon medical proof of disability.

Separate from WSIB you may be entitled to additional benefits from our Group Disability Income Plan. To avoid a delay in processing, we recommend that you complete this form in accordance with the instructions and promptly forward it to:
